Ahmed Glaucoma Valve Implantation in Treatment Resistant Glaucoma

Purpose: To evaluate the efficacy of Ahmed Glaucoma Valv implantation in treatment-resistant glaucoma patients
Material and Methods: Between January 2004-December 2009, patients with glaucoma followed up with medical treatment, despite intraocular pressure (IOP) higher than 20 mmHg with 15 men and 12 women, 27 patients of 27 eyes, Ahmed glaucoma valve (AGV) implantation was performed in our clinic, glaucoma unit. All patients age, gender, previous ocular surgery, type of glaucoma, preoperative and postoperative visual acuity, preoperative IOP, postoperative IOP, number of drugs used before surgery were recorded. Success after surgery was defined as IOP below 20 mmHg with an addition of antiglaucoma medications or without medical treatment, and the failure was defined as IOP being over 21 mmHg with additional treatment.
Results: Mean age of patients were 52.37±18.62 years. Before surgery 7 (25.9%) patients were using 2 drugs, while 18 (66.7%) were three-drugs and 2 (7.4%) were using four drugs. Twelve patients' (44.4%) preoperative visual acuity were hand motion, four patients' (14.8%) were one cfp, two patients' (7.4%) were 2 cfp, five patients' (18.5%) were p (+) and four patients' (14.8%) were p ( -), respectively. After surgery, first day mean IOP was 14.59 ± 8.51 mmHg, first month 17.48±6.62 mmHg, first month 18.11±7.09 mmHg, 6th month 20.78±6,78 mmHg, and 12th month 18.55±6.95 mmHg, respectively.
Conclusion: AGV implantation is a safe and effective method for the control of IOP in treatment-resistant glaucoma patients.
