Refractive lens change is a treatment modality for presbyopia treatment. With the advances in surgical technics and the increase in population who have presbyopia, the use of intraocular lenses (IOL) became crucial. Many different lenses including multifocal IOLs are implanted to treat presbyopia. None of the IOLs were found to be ideal. Currently accommodative intraocular lenses (AIOL) are under renewal. These lenses use the performance of ciliary body. Two main design concepts exist. First axial shift concept relies on anterior movement of one or two optics creating accommodation ability. Second, relies on the change of IOL curvature or shape. Although the experiences with the fi rst AIOL Crystalens were disappointing, the reported results with the new AIOLs are better. Sulcus is the place preferred for AIOL implantation instead of capsular bag in new design AIOLs. In newer design AIOLs toric ability may be involved.
